Heart Hugs
Paradigm Health Heart Hugs Program
Comfort, Connection, and ongoing Support
Heart Hugs supports children, teens, and their caregivers who are impacted when a loved one is receiving hospice care. Support begins while the patient is on hospice and continues after death for as long as the family needs it. Families with children are automatically identified for the program, which is offered in partnership with Brooke’s Place to provide compassionate, age-appropriate grief support.

Caring Support for Children and Families
Heart Hugs is part of Paradigm Health’s bereavement program, created to support children, teens, and their caregivers when a loved one is receiving hospice care. Families with children are automatically identified for Heart Hugs, and your care team coordinates the delivery of comfort items and ongoing support. Care begins while your loved one is on hospice and continues after death for as long as your family needs support. Through partnerships with Brooke’s Place in Central Indianapolis and Erin’s House in Fort Wayne and surrounding communities, families also have access to specialized grief support programs, counseling resources, and community-based services designed specifically for children and caregivers navigating loss.

“Being around other teens who had lost someone too made it easier to talk about how I was feeling. It helped knowing I wasn’t the only one going through it.”
