Kelly Streza is dedicated to educating and supporting patients and their families as they navigate their unique healthcare journeys and identify individual goals of care. A proud lifelong resident of the Tipton area, Kelly continues to live and work in the community she serves, providing exceptional hospice and palliative care to Kokomo and surrounding regions, including Howard, Tipton, Clinton, and Boone Counties.

As a Transitional Care Navigator, Kelly meets with patients and their families to gain a comprehensive understanding of their healthcare stories. She uses this holistic approach to assess mental or physical declines, identify increasing hospital visits, and explore ways to streamline doctor’s visits and medication plans. She offers tailored recommendations for care and connects families to valuable resources to make the healthcare journey more manageable.
