As the nation celebrates Nurse’s Week 2024, Paradigm Health takes the time to celebrate our own hospice care nurse team with a look at the above-and-beyond hospice care and support they provide hospice patients and their families.  

You might wonder what role a hospice care nurse provides to individuals at end-of-life no matter where they call home. Beyond a priority to keep patients comfortable their role may include: 

Plan of Care 
Our hospice nurses initially work to asses our patient’s needs to create an individual and unique plan of hospice care that takes into account the physical, emotional and spiritual needs and goals of each patient.  

Managing Pain 
Paradigm Health’s medical director works closely with our nurses to develop a pain management plan that is specific to each individual patient’s needs that may or may not include medications and other therapies. 

Understanding Hospice Symptoms 
Many hospice patients experience a wide range of symptoms that come alongside the hospice journey including pain, nausea, anxiety and shortness of breath. Paradigm Health nurses will identify these symptoms and recommend effective solutions to keep our patients as comfortable as possible.  

At Work for the Patient 
Paradigm Health’s holistic community-based approach to hospice care promotes the goals and needs of the patient and advocates for their needs and works to ensure that their wishes are carried out. 

Beyond Patient Care  
Our nurse care extends beyond the patient as a support team for the patient’s family and caregivers to provide education and resources that can help them cope with the stress and emotions of a loved one’s hospice care journey.
