Paradigm Health in Hamilton County, Indiana

Q: Dee Ann, talk a bit about your career path and how you came to work or hear about Paradigm Health.
A: I began my career in the 1980s in healthcare and consumer and pharmaceutical sales. After a company downsize, my career path shifted into long-term care. I also worked as a marketer liaison for a mobile physician group that made house calls, a home health care representative, and shifted to a role in hospice in 2017. About a year ago, after serving in some nonprofit support roles, I realized I missed hospice care and became a transitional care navigator with Paradigm Health.
Q: What about Paradigm Health is unique from other hospice and palliative care companies?
A: Paradigm Health is unique in that it has the ability to pivot and do the right things for the right reasons as a company mission and that directly translates to above-and-beyond care for our patients and their families.
Q: Explain what a transitional care navigator does and how it serves patients and their families.
A: At Paradigm Health, the transitional care navigator position is ideal for me as I’m able to use my previous hospice and long-term care knowledge when meeting with patients and their families to discuss the benefits of hospice and palliative care. I’ve lived and worked in Hamilton County for over 20 years and I am familiar with a majority of our referral sources and to work and serve those I consider neighbors.
Q: When will patients and families know it is time to seek hospice or palliative care and what should they be looking for to identify a need for additional support?
A: While every patient and family’s end-of-life journey is different, I share with them that Paradigm Health is here to meet them wherever they are. Declines that many individuals and families notice can be physical, cognitive and/or both so whenever they seek hospice or palliative care is usually the right time for them.

Q: Talk about the Paradigm Health community-based or team-approach to care.
A: The Paradigm Health culture and mission is to raise the bar of the hospice and palliative experience one family, one team member, one community at a time. Hamilton County is my community.
Q: Share what is most rewarding to you in working for Paradigm Health and ultimately their patients and families/referral sources.
A: The Paradigm Health team always feels valued and heard and that directly translates to the culture of care we provide to patients and families every day. We value them, we hear them, and we support their individual goals and wishes. We don’t provide a cookie cutter approach to care but are intent on supporting patients and their families as they navigate their hospice and palliative care journeys.
