In Central Indiana, Paradigm Health is a leader and innovator as a hospice and palliative care provider. Already established as a compassionate end-of-life hospice care provider, Paradigm Health was the first in the nation to receive accreditation by the  Accreditation Commission for Health Care (ACHC)  for our community-based approach to palliative care. Our services can help referral sources connect patients with our hospice and palliative care services in real-time and with responsive intake assistance and guidance.  

Every Day Matters

While many associate  hospice care  with end-of-life care, at Paradigm Health we know it is really about  living—and doing so with services that enrich each and every day’s opportunity to celebrate life. It is about caring for the individual in a respectful, comforting and unique way that maximizes and nurtures their life experiences. 

Who is Eligible?
Individuals are eligible for hospice care are typically seeking end-of-life care within the last six months of life. 

When is Hospice Care Needed?
Criteria for patients to qualify for hospice care utilize these basic guidelines. These guidelines—provided as a convenient tool and not as a replacement for a physician’s professional judgment—help physicians determine when their patients meet clinical guidelines for hospice eligibility for life-limiting illnesses. 

  • The illness is terminal (a prognosis of less than 6 months) and the patient and/or family has elected palliative care. 
  • The patient has an observable and documented deterioration in overall clinical condition in the past 4-6 months indicated by: 
    • More than three hospitalizations or emergency room visits 
    • A decrease in tolerance for physical activity 
    • Decrease in cognitive ability 
    • Other comorbidity conditions identified by a physician or caregiving team 
Working Together As A Team

At Paradigm Health, palliative care  is a collaboration. Having an advocate to help partner, support and lead a patient’s palliative care can be valuable and allow for a streamlined, community-based approach. Paradigm Health acts as an extra layer of hands-on care to assist in patient care but does not replace a patient’s primary care physician or duplicate efforts. 


What is Palliative Care?

Palliative care is offered to patients who are seriously ill to be used alongside curative and other treatments.  It does not replace care by a family care physician, but rather addresses symptoms and care management.  Palliative care is also not dependent on a set criteria or treatment options and can provide patients and families with assistance for advanced care planning and help guide goals of care discussions.  Illnesses most commonly treated by a palliative care team are heart disease, cancer, stroke, diabetes, renal disease, Parkinson’s and Alzheimer’s disease. 

Our Continuum of Care

Knowing the difference between hospice and palliative care can be confusing. For Paradigm Health patients can rest assured that their palliative care can transition to hospice care seamlessly should it be needed in their continuum of care. Palliative care is often a step toward hospice care, but it is not the same as hospice care. Patients on palliative care are within one to five years of needing hospice care while hospice patients are typically seeking end-of-life care within the last six months of life.
