The First Choice for Hospice and Palliative Care Referrals

In Central Indiana, Paradigm Health is a leader and innovator as a hospice and palliative care provider. Already established as a compassionate end-of-life hospice care provider, Paradigm Health was the first in the nation to receive accreditation by the Accreditation Commission for Health Care (ACHC) for our community-based approach to palliative care. Paradigm Health’s mission is to provide above and beyond care for its patients and their caregivers and families.

We call Central Indiana home too.

As an Indianapolis-based hospice and palliative care provider, we are caring for and supporting your neighbors—and our own. Our services can help referral sources connect patients with our hospice and palliative care services in real-time and with responsive intake assistance and guidance.

Paradigm Health understands that strong referral partner relationships are integral to successful, compassionate caregiving for our shared patients and families. Responsive turnaround times, resource-providing expertise, and a commitment to value create shared visions and philosophies around providing above and beyond care for our patients and providing our referral partners with the same commitment and high quality of service.

Our founder Jeff Jarecki has always said “If we strive to become the employer of choice in support of our team, that feeling of compassion for what we do directly translates to exceptional hands-on, patient caregiving making Paradigm Health the provider of choice for so many of our referral partners.”

A Responsive and Holistic Approach to Caregiving
The Importance of Early Referrals

According to the National Hospice and Palliative Care Organization, of the 1.6 million Americans who received hospice care, more than half received care for less than 14 days and a third of patients were on hospice care less than 7 days. Many families often express they wished they had engaged in the hospice care conversation earlier than they did. As a referral partner – you can make a difference in the hospice care journey for your patient as well.

Benefits of an early hospice care referral can:

  • Enhance the quality of life
  • Establish a continuum of care from palliative to hospice care by one provider, Paradigm Health
  • Begin conversations about putting in place goals of care
  • Allow for supportive care with input from a primary care provider or specialist
  • Create discussion around advanced directives
Our Continuum of Care

Knowing the difference between hospice and palliative care can be confusing. For Paradigm Health patients can rest assured that their palliative care can transition to hospice care seamlessly should it be needed in their continuum of care. Palliative care is often a step toward hospice care, but it is not the same as hospice care. Patients on palliative care are within one to five years of needing hospice care while hospice patients are typically seeking end-of-life care within the last six months of life.
