The Importance of Early Referrals

According to the National Hospice and Palliative Care Organization, of the 1.6 million Americans who received hospice care, more than half received care for less than 14 days and a third of patients were on hospice care less than 7 days. Many families often express they wished they had engaged in the hospice care conversation earlier than they did. As a referral partner – you can make a difference in the hospice care journey for your patient as well. 

Benefits of an early hospice care referral can:

  • Establish a continuum of care from palliative to hospice care by one provider, Paradigm Health 
  • Begin conversations about putting in place goals of care 
  • Allow for supportive care with input from a primary care provider or specialist

Our Continuum of Care

Knowing the difference between hospice and palliative care can be confusing. For Paradigm Health patients can rest assured that their palliative care can transition to hospice care seamlessly should it be needed in their continuum of care. Palliative care is often a step toward hospice care, but it is not the same as hospice care. Patients on palliative care are within one to five years of needing hospice care while hospice patients are typically seeking end-of-life care within the last six months of life.
